1、到成都市人民南路二段65号省旅游大楼五楼，就是旅游局，报名人员到旅游局领取并认真填写《全国导游人员资格考试报名表》，由本人所在单位人事部门（待业人员由户口所在地街道办事处）审核并加盖公章。
报名时须交已盖章的《报名表》、本人一寸正面半身免冠照片三张，并出示身份证、学历证明、国家县（区）一级医院出具的近期健康状况证明（含肝功及两对半检查）。
